The surgical management of venous disease is filled with dichotomies. Procedures span the spectrum from the most mundane saphenous vein stripping (one of the first procedures I did as an intern) to sophisticated central venous reconstructions. Venous surgery tends not to be glamorous, playing second fiddle to arterial reconstruction but frequently is technically more demanding and very dependant on meticulous technique. Diagnosis may be straightforward as in identifying lower extremity varicosities or extremely challenging as in confirming a true thoracic outlet syndrome. Though responsible for a great deal of morbidity in the population as a whole, it receives less attention than it deserves from caregivers. The patient who presents with DVT now frequently is sent home immediately on injectable anticoagulants “per protocol,” deceptively minimizing the long-term sequelae of chronic venous stasis disease. Venous disease is the Rodney Dangerfield of surgery, getting little respect. This is a mistake as Dr. Illig and his colleagues capably illustrate in this issue of Operative Techniques in General Surgery. They show the numerous surgical options available to treat various venous disorders and the evolving nature of those treatments. This broad reaching issue spans newer treatments for the lowly varicosity all the way to the complexities of vena cava and subclavian vein reconstruction. Just as tackling venous disease takes a special kind of surgeon with a special commitment to succeed, so did Dr. Illig take on the challenging task of organizing this issue, something that a less motivated individual might shy away from. Indeed, measles virus was the first virus to be associated with immunosuppression with many of the manifestations to be observed one hundred years later with HIV infection. Measles is also associated with development of both post-infectious encephalomyelitis, an autoimmune demyelinating disease, and subacute sclerosing panencephalitis, a slowly progressive neurodegenerative disorder. How measles virus infects cells, spreads to various tissues and causes disease, as well as the role of the immune response, generation of new vaccines, and use as a vector for gene delivery are topics covered in these two volumes.

Cardiac resynchronization therapy is a new therapy for advanced heart failure patients. This therapy has been shown to improve quality of life, exercise capacity, NYHA classification, and even reverse the detrimental "reverse remodeling" that occurs in advanced heart failure - in patients already receiving optimal pharmacological therapy.

The typical patient has NYHA III/IV HF symptoms, a QRS > 120 ms, LVEDD > 55 mm, without any indication required for pacing. The therapy is available in an ICD device, too. CRT has been shown to be effective in patients with both ischemic and non-ischemic cardiomyopathy.

The implant procedure is part of a learning curve. Many physicians who already perform electrophysiology procedures, or PPM, or ICD implants are the ideal implanter - but are surprised at the challenges this implant presents. This book will help "flatten" the learning curve for new physician implanting, and will provide a "guide' for those who have already started implanting.

The success rate is about 90o after 10 years of application, we have found that there are still patients who do not get access to this great therapy because of the complexity of the patient's anatomy, or physician skills.

The book is written by three authors with a great deal of experience culminating in over 3000 cases between them.

This book is intended for cardiologists who currently implant either or both pacemakers and cardioverter defibrillators with cardiac resynchronization therapy (also called bi-ventricular pacing), or plan to implant in the future. This book will be extremely useful for the new cardiologist in training who is learning to implant devices.
